Notice of Privacy Practices

Pre-launch notice: Unified Healing Co. is not yet an operating medical clinic. We are currently collecting interest-list information only (name, email, phone, zip code, and general treatment interest). We do not currently collect, store, or process protected health information (PHI). This privacy policy will be updated prior to the clinic opening to reflect our full HIPAA Notice of Privacy Practices once clinical operations begin.

About This Notice

This Notice of Privacy Practices describes how your personal information may be used and disclosed by Unified Healing Co., and how you can access and control your information. Please review it carefully.

As required by the Privacy Regulations promulgated pursuant to the Health Insurance Portability and Accountability Act of 1996 (HIPAA), this notice will be updated to include our full clinical privacy practices once the clinic is operational and treating patients.

Information We Currently Collect

During the pre-launch phase, our website collects the following information through our interest forms:

  • Contact information: first name, last name, email address, phone number
  • Location information: zip code, city or town
  • General treatment interest: conditions of interest, treatment types of interest
  • Insurance carrier or payment preference (carrier name only, no policy numbers or member IDs)
  • General clinical history: whether you have tried two or more antidepressants, whether you have a doctor referral
  • How you heard about us
  • Any questions or comments you choose to share

For referring providers, we also collect: professional credentials, specialty, practice name, practice location, and referral partnership interest.

What we do NOT collect at this time: We do not collect Social Security numbers, insurance policy numbers, member IDs, dates of birth, medical record numbers, diagnostic codes, prescription information, insurance card images, or any other protected health information. Our interest forms are not a medical consultation, intake process, or request for treatment.

How We Use Your Information

Information collected through our interest forms is used solely for the following purposes:

  • To confirm receipt of your inquiry and communicate with you about our clinic's opening
  • To send periodic updates about the clinic's progress toward opening
  • To respond to your questions or comments
  • To understand geographic demand and treatment interest in the communities we plan to serve
  • For referring providers: to coordinate future referral partnerships once the clinic is operational

We will never use your information for unsolicited marketing unrelated to Unified Healing Co.'s services. We will never sell, rent, or share your personal information with third parties for their marketing purposes.

How We Store and Protect Your Information

Your information is stored within our customer relationship management (CRM) platform, which uses industry-standard encryption in transit (TLS/SSL) and at rest. Access to your information is restricted to authorized members of our team only.

While no method of electronic transmission or storage is completely secure, we take reasonable precautions to protect your personal information from unauthorized access, use, or disclosure.

Information Sharing and Disclosure

We do not share your personal information with third parties except in the following limited circumstances:

SMS Text Messaging

If you provide your phone number through our forms, you may receive SMS text messages from Unified Healing Co. related to your inquiry, clinic updates, or scheduling information. By providing your phone number, you consent to receive these messages. Message and data rates may apply.

You can opt out of SMS messages at any time by replying STOP to any message you receive from us. SMS opt-in or phone numbers will not be shared with or sold to any third parties for their marketing purposes.

  • The right to inspect and copy your protected health information
  • The right to request restrictions on how your PHI is used or disclosed
  • The right to receive confidential communications by alternative means or at alternative locations
  • The right to request amendments to your health information
  • The right to receive an accounting of certain disclosures of your PHI
  • The right to obtain a paper copy of this notice
  • The right to file a complaint if you believe your privacy rights have been violated

We reserve the right to update this privacy notice at any time. Changes will be posted on this page with an updated effective date. Significant changes to how we handle your information will be communicated via email to individuals on our interest list.
